It is strange that my doctor gave three prescriptions today, one is for far distance, one is for computer and the other is for reading. And as a matter of fact, I only need two pairs of progressive bifocal glasses, one is for Distance/Reading use, and the other is for Computer/Reading use. Unfortunately, I don’t know how to convert this prescription and fill out them online. The detailed information of my eye glasses prescription is as follows:

Since I was not sure how to input theses prescription information, I called 877-893-7315 which is the phone number of the customer service of www.glassespeople.com where I have been recommended to purchase prescription glasses from. Their helpful customer service rep have worked out and formalized the following two prescriptions I need.
Distance/reading prescription:
SPH CYL AXIS ADD
O/D +1.00 +0.50 075 +2.25
O/S +1.25 +0.25 170 +2.25
And here is my computer/ reading prescription:

O/D +2.25 +0.50 075 +1.00
O/S +2.50 +0.25 170 +1.00
I realized that it is too simple to convert my eyeglasses prescription. Next time, I will work it out by my self. If I need bifocal lenses for distance and reading use, I could just simply use the SPH of my reading only prescription to deduct the SPH value in my distance prescription to get my ADD value which is the reading addition strength. And if I need bifocal glasses for computer and reading use, just use the same SPH in my reading only prescription to deduct the SPH value in my computer prescription to get ADD value.
